Basic Carrier Setup Requirements
Provide a valid MC/MX or USDOT number for registration.
Intrastate carriers without a DOT number will need to provide a state registration number.
Complete a Broker-Carrier Agreement with an authorized M&J Transport member.
Provide a valid Tax Identification Number (TIN) and federal tax information.
Provide a certificate of insurance that meets the minimums below.
Auto Liability/BIPD - $1,000,000
Workers’ Compensation - must meet statutory limits
Cargo Liability - $100,000
